Adeshalewa Adenoyo - Opakunle, NP-C

Shalewa, known to her patients as Shelly, became a member of the IHMG team in 2017, and is a part of the CLARITY and SNFist service lines. She has worked as an Advanced Practice Nurse since 2007. Shalewa graduated from Purdue University Calumet and is a member of the American Academy of Nurse Practitioners. She started her practice in a Primary Care setting and later transitioned to providing care to clients in Post-Acute Care/ Skilled facilities, collaborating to decrease hospital re-admission. Shalewa continues to be committed to providing compassionate, high-quality care throughout all the stages of life.

Amy Buchenau, FNP-BC

Amy became a part of IHMG in 2012. She is a part of the ATLAS service line. Amy earned her undergraduate degree from Luther College. She then worked in acute care nursing for over 20 years. This includes experience in emergency and trauma care. She went on to get her Master of Science in Nursing degree at the University of Illinois at Chicago. She is also a board-certified Family Nurse Practitioner.
Angela Giardono, BSN, MS, FNP-BC

Angela started with IHMG in 2015. She is a part of the ATLAS service line and the CLARITY service line. She graduated from the Saint Anthony College of Nursing in 2004 with her Bachelor of Science in Nursing degree. In 2015, she again graduated from Saint Anthony College of Nursing, this time with her Master of Science in Nursing degree. Previous experience for Angela includes being an Emergency Medicine Nurse Practitioner.

Annie has been working with IHMG in the ATLAS service line since 2016. She earned her Bachelor of Science in Nursing degree from Aurora University and has a diverse range of experience from newborn care to geriatrics as a nurse for 19 years. She graduated with her Master of Science in Nursing degree in 2016 from Olivet Nazarene University. She is board-certified in Family Practice through the American Nurses Credentialing Center. Anna is also a Certified Health Coach and is committed to serving her community on health education and disease prevention.
Beth Rieches, FNP-BC

Beth came onboard IHMG in 2012, and works in COMPASS, FUSION and ABODE service lines. She graduated from Northern Illinois University in 2009 with a Master of Science. Beth is certified with the American Heart Association in the Advanced Cardiovascular Life Support Program. Her specialties include post-operative care to patients who have undergone cardiovascular surgery as well as geriatrics. She is trained to provide full primary care, order laboratory and diagnostic testing, and prescribe appropriate medications based on disease status.

Caroline is a part of the CLARITY service line and has been with IHMG since 2015. She’s an Adult Nurse Practitioner and is certified through the American Academy of Nurse Practitioners. Caroline became a Licensed Nurse Practitioner in 2001; she would go on to earn her Bachelor of Science in Nursing degree in 2006, and her Master of Science in Nursing degree in 2010.

Courtney began working with IHMG in 2016, in ATLAS, SNFist and ABODE service lines. She obtained her Bachelor of Science in Nursing degree with a minor in Intercultural Studies from Indiana Wesleyan University in 2011. Courtney then went on to get her Master of Science in Nursing degree with a focus on family practice from Northern Illinois University in 2016. She is certified by the American Academy of Nurse Practitioners. She has training in alternative therapies in family practice, is a Certified Lactation Educator, and is Nexplanon certified for insertion and removal. Courtney has previous experience within progressive care, pediatric Intensive Care Unit, Neonatal ICU, and Pediatrics.
Cynthia Hammond, NP-C

Cynthia has been a part of IHMG since 2016, working in the ATLAS service line. Prior to IHMG she worked for a company that specialized in geriatric care. She became a Licensed Practice Nurse in 1998, a Registered Nurse in 2005, graduated with her Bachelor of Science in Nursing degree in 2011, receiving her Master of Science in Nursing degree in 2015. She is currently working on becoming a Wound, Ostomy and Continence Nurse and has special training in wound care. Cynthia also participates in medical missions to locations like Honduras and Haiti.
Danielle Jordal, MSN, ACNP-BC

Dani has been with IHMG since 2015 and works with Swedish American Hospital. She graduated with her Master of Science in Nursing degree in 2012 from University of Illinois at Chicago. She is certified by the American Nurses Credentialing Center as an Acute Care Nurse Practitioner. She holds certification in Advanced Practice Cardiometabolic Nursing. Dani specialized in the management of cardiac patients with experience in both acute and chronic cardiac conditions as well as chronic diseases. She takes pride in providing clinically competent care, treatment plans and education of disease processes to both her patients and their families. She is trained to provide full primary care, laboratory and diagnostic testing, and prescribe appropriate medications based on disease states.
Elaina Parlapanis MSN, APN, NP-C

Elaina joined IHMG in 2016 as a part of the ATLAS service line. She graduated with her Master of Science in Nursing degree in 2015 from Governor’s State University and is currently pursuing her Doctorate of Nursing Practice degree from the University of Alabama. Elaina is certified by the American Academy of Nurse Practitioners as a Family Nurse Practitioner. She’s gained a diverse range of experience within her 10-year nursing career, attaining knowledge from many specialties including Cardiology, Intensive Care, Oncology, Rehabilitation, and Neurology at state of the art facilities within the Chicagoland area. Elaina is trained to provide full primary care, order laboratory and diagnostic testing, and prescribe appropriate medications based on disease status.

Liz has been with IHMG since 2011. She is a part of the COMPASS program. Liz did her undergraduate work at Miami University of Ohio. She obtained her Master of Science in Nursing degree from the University of Massachusetts. Liz has worked in bone marrow transplant and as a hospitalist. She has clinical interests in geriatrics and the management of chronic diseases.

Heather has been with IHMG since 2015, and is a part of the CLARITY service line. She is credentialed through the American Nurses Credentialing Center. Heather received her Bachelor of Science degree from Western Illinois University in 1997. She later went on to graduate from Cameron University in 2000 with a Master of Science in Behavioral Sciences/Psychology degree. Then in 2010, Heather graduated from University of Illinois at Chicago with a Master of Science in Nursing degree.

Jaycie started with IHMG in 2017. She currently is a part of ABODE, COMPASS and CLARITY. Jaycie graduated from Illinois State University in 2004 with a Bachelor of Science in Nursing degree. In 2010, she again graduated, but with a Master of Science in Nursing degree from the University of Illinois at Chicago. Jaycie is an Adult Geriatric Nurse Practitioner, and is certified by the American Nurses Credentialing Center. Jaycie has experience teaching nursing students, which includes being a Preceptor for Advanced Practice Nursing student at Loyola University as well as Mennonite College of Nursing. She was also the recipient of the Geriatric Award from Mennonite College of Nursing in 2004.

Jody joined IHMG in 2016 and currently works in ABODE and SNFist service lines. She began her nursing career at Rock Valley College, graduating in 2010 as a Registered Nurse. Jody then moved on to Chamberlain College of Nursing and graduated in 2013 with a Bachelor of Science in Nursing degree. In 2016, she graduated from Chamberlain College of Nursing with a Master of Science-Family Nurse Practitioner degree. Jody is credentialed through the American Nurses Credentialing Center.
Judy Sargent, AGPCNP-BC

Judy has been with IHMG since 2017.  She works with SNFist, ABODE, and transition management lines of care. Judy is a Nurse Practitioner who graduated with her Masters in Nursing degree from North Park University in Chicago in 2015 following the Adult-Gerontology primary care track.  Judy has nursing degrees from The College of Lake County which she received in 2007, and from Olivet Nazarene University in 2012. Judy’s certification is from the American Nurses Credentialing Center as an Adult-Gerontology Primary Care Nurse Practitioner. Judy also maintains a certification from the American Heart Association in Basic Life Support, Cardiopulmonary Resuscitation and use of an Automated External Defibrillator. She has been caring for our senior population since 2007.  Judy is a member of the Illinois Society of Advanced Practice Nurses, American Association of Nurse Practitioners, and the American Nurses Association.

Judy has been with IHMG since 2016. She comes to IHMG with 36 years of clinical experience and spent the last 11 years as a Family Nurse Practitioner in settings such as Emergency Medicine, Primary Care, Occupational Health Medicine, Clinical Service Manager, and most recently in SNFist. Judy is currently a part of the ATLAS and CLARITY service lines at IHMG. Judy completed her Associates Degree in Nursing at Truman College of Nursing in 1980, Bachelor of Science in Nursing at St. Xavier’s University in 2003, Family Nurse Practitioner degree at Northern Illinois University in 2005, and most recently in 2015, her Doctorate of Nursing Practice degree through St. Francis Medical Center College of Nursing.

Katie has been with IHMG since 2010, and works with COMPASS. She graduated with her Master of Science in Nursing degree in 2005 from the University of Illinois-Chicago. She is certified by the American Nurses Credentialing Center as an Acute Care Nurse Practitioner. She is also certified in Basic Life Support, and Advanced Life Support from the American Heart Association and in critical care from the American Association of Critical Care Nurses. She is a member of the American Association of Critical-Care Nurses, Society of Critical Care Medicine and American Academy of Nurse Practitioners. She is a recipient of the 2011 Illinois Nurses Association "Clinical Excellence" award. She has a clinical interest in geriatrics, dementia care, and the management of chronic diseases.

Kathy joined IHMG in 2015 with almost 20 years’ experience as a critical care Registered Nurse. She works with the ATLAS service line. Kathy has her Master of Science in Nursing degree and is certified by the American Academy of Nurse Practitioners as a Family Nurse Practitioner.
Kelly Brendel, FNC-BC

Kelly has been with IHMG since 2016, and works with COMPASS, and LTC/SAR. She completed her undergraduate degree at St. Anthony College of Nursing. She worked for 12 years in the Neuro Trauma Intensive Care Burn Unit before going back to St. Anthony College of Nursing to complete her Master of Science in Nursing degree. Kelly is currently finishing her Doctorate of Nursing Practice degree. Kelly believes that each interaction is a unique and rewarding experience and an opportunity to provide holistic care, health promotion and symptom management to individuals, families and the community. Kelly’s background and interests include appraising health risks, health promotion, appropriate evaluation, testing and medications management, health education and preventative care for patients of all ages.
Kisha Redd, FNP-BC

Kisha works in the COMPASS service line and joined IHMG in 2016. She’s a certified Family Nurse Practitioner. Kisha obtained her Bachelor’s in Health Administration degree from Northern Illinois University in 2001, and her Bachelors of Nursing degree in 2006, also from NIU. In 2013, Kisha graduated from NIU once more, with a Master of Science degree in Nursing.
Lauran Wendt, FNP-BC

Lauran is a part of the SNFist service line and has been with IHMG since 2013. She is also a 2013 graduate of St. Anthony College of Nursing. She earned her Master of Science in Nursing degree and became certified as a Family Nurse Practitioner. Prior to that, Lauran worked as a Registered Nurse for nearly 10 years. Her experience includes working with patients in sub-acute rehabilitation facilities where she educated patients on their diagnoses and assisted in medication administration and treatment. Additionally, Lauran has experience working with the homebound community through home care services where she provided wound care and home infusions.
Lisa Antle, PhD, RN, GCNS-BC, APNP

Lisa joined IHMG in 2017 and is a part of the SNFist service line working primarily in our Wisconsin facilities. She received her Bachelor of Science in Nursing from the University of Wisconsin-Madison, Master of Science in Nursing with an emphasis in gerontological research from St. Louis University, and PhD from the University of Wisconsin-Milwaukee in nursing. She has taught various levels of nursing education and practices as a Gerontological Advanced Practice Nurse. Lisa received a Wisconsin Space Consortium Fellowship to conduct independent research in the treatment of osteoporosis at the NASA Ames Research Center. Lisa’s ultimate goal for this program is to mentor interdisciplinary clinical students and investigate interventions for the prevention of osteoporosis. Some other research interests of Lisa’s include investigating the role of oxidative stress and apoptosis associated with progressing frailty in the older adult population.

Lori has been a part of IHMG since 2015 and works within the CLARITY service line. She’s been a psychiatric nurse practitioner for the past 30 years, working in various clinical and managerial arenas. For the last 15 years, she has been working almost exclusively in the geriatric psychiatric population. She also has been active in educating staff on the care of patients with dementia. Lori received her Master of Science in Nursing degree from Boston College. She is a Charter member of the Illinois Psychiatric Nursing Association and a member of the American Psychiatric Nursing Association.

Ellen joined IHMG in 2014 as a part of COMPASS and CLARITY service lines. She is an Advanced Practice Nurse with over 10 years of nursing experience which includes work in allergy/asthma/immunology, adolescent health, women’s health, and geriatrics. Ellen graduated from Frontier Nursing University in 2013 with her Master of Science in Nursing degree and is board-certified by the American Nurses Credentialing Center as a Family Nurse Practitioner. She specializes in providing high quality, cost effective primary care in unique settings such as patient’s homes, assisted living and long-term care facilities. She also has experience in sub-acute rehabilitation with the goal to reduce hospital stays and readmissions and focus on health maintenance, chronic disease management, and education. Ellen is committed to serving the community with an overarching focus on the merging of wellness promotion and illness prevention.

Megan works in the COMPASS, ABODE and ATLAS service lines and has been with IHMG since 2012. She earned her undergraduate degree from Rock Valley College and Olivet Nazarene University. She has worked in trauma/emergency, immediate care and several other departments as a nurse for over 20 years in the Rockford area. She went on to get her Master of Science in Nursing degree from the University of Illinois at Chicago. She is a board-certified Family Nurse Practitioner.

Mercy Abunaw AANP, FNP-C

Mercy started with IHMG in 2017, and is a part of the CLARITY service line. She has extensive experience in health care, including being a wounds treatment nurse, Staff Nurse, Hospice and Alzheimer’s Disease Unit Manager. She received her Certified Nursing Assistant certification in 2003. Mercy obtained her Bachelor of Science in Nursing degree in 2007 from Danville Community College. She worked as staff, a nurse, a nurse manager, and Director of Nursing for many years in the long-term care setting before graduating with a Master of Science in Nursing degree from Governor’s State University in 2016. Mercy is very active in doing volunteer work, like administering vaccines to low income neighborhoods in Danville, IL. She also has a Bachelor of Arts degree in English/Journalism.

Molly has been with IHMG in the ABODE service line since 2015. She began her nursing career in 1995 when she graduated from Illinois Wesleyan University with a Bachelor of Science in Nursing degree. She has experience in medical, oncology, trauma and intensive care, and hospice nursing. In 2015, she graduated with honors from the University of Illinois at Chicago, earning a Master of Science degree. She is board-certified as an Adult-Gerontology Primary Care Nurse Practitioner by the American Nurse Credentialing Center. She takes pride in the care she provides to her patients and their families, especially those who have chronic medical conditions that prevent them from leaving their homes for routine medical care. Molly guides her care and treatment using evidenced based practice guidelines to provide the most relevant and appropriate treatments to her patients. She has special interests in geriatrics, dementia, and end-of-life care.
Nicole Gonzalez, MSN, APN, FNP-BC, CCRN

Nicole became a part of the IHMG team in 2017 and works in the CLARITY service line. She is certified through the American Association of Critical-Care Nurses as a Family Nurse Practitioner. Nicole received her Associate of Science degree in 1999, Associate of Arts degree in 2000, Bachelor of Science in Nursing degree in 2003, and her Master of Science in Nursing degree in 2015. She has training in Basic Life Support, Advanced Care Life Support, National Institute for Health stroke certification, joint injections, trigger point therapy and is also bilingual in English and Spanish. Nicole previously had a whole nursing career, and was a Rapid Response and Code Blue team member. Nicole was also a recipient of the Sister Mary Linus Nowak Nursing Research Award. She has interests in vulnerable populations health, critical care medicine, geriatric health and aromatherapy.

Rachel is a part of CLARITY, ABODE, and RRP service lines at IHMG, having joined the IHMG team in 2012. She graduated in 2009 from St. Anthony College of Nursing with a Master of Science in Nursing degree. She has extensive experience in cardiology and has previously worked in a congestive heart failure clinic. She also spent time managing chronic diseases for homebound patients. Rachel enjoys working with our geriatric population and is known for her excellent management and communication skills with patients and families.

Renee has been a part of the IHMG team since 2015 and she works within the ATLAS service line. Renee received her Master’s degree in Nursing in 2014. Sarah Maas, FNP-BC

Sarah has been with IHMG since 2016, as a part of CLARITY and SNFist service lines. In 2005, she graduated with a Bachelor’s degree from University Illinois in Psychology and would go on to get her Bachelor’s in Biology degree from Illinois State University. She earned her Bachelor of Science in Nursing from Mennonite College of Nursing’s accelerated program in 2008. She worked as a hospital nurse in neuro specialty care and progressive care units. In 2014, she completed her Master of Science in Nursing degree at Mennonite College of Nursing and is board-certified by the American Nurses Credentialing Center as a Family Nurse Practitioner. She specializes in providing medical management for patients in sub-acute rehab, long term care and assisted living facilities.
Tarah Gibbs, NP-C

Tarah joined IHMG in 2017 and is a part of the SNFist service line. She is a Family Nurse Practitioner with experience in family medicine, integrated medicine, and physical medicine. She graduated Suma Cum Laude from Methodist College of Nursing Class of 2008 and University of Illinois at Chicago, Class of 2016. Tarah started her nursing career at Pekin Hospital as a labor and delivery nurse. In 2012, Tarah accepted a position at Buehler Endowment Home to develop a Home Healthcare Service for the assisted and independent residents and coordinate Care Plans. Upon graduation from University of Illinois at Chicago in May of 2016, she took a position at Preferred Care Medical Center in Eureka, Illinois specializing in integrated medicine, ambulatory rehabilitation and chiropractic care. Tarah currently teaches nursing obstetrics clinical at Bradley University in Peoria Illinois.

Teti has been with IHMG since 2016 and is a part of the ATLAS service line. She became a Licensed Practice Nurse in 1996, from Dawson Technical and a Registered Nurse in 2003 from Kennedy King College. In 2011, she received her Bachelor of Science in Nursing degree from Governor’s State University. In 2013, she obtained her Master of Science-Family Nurse Practitioner degree also from Governor’s State University.
