On-Site Primary Care Providers

As most healthcare administrators know, five percent of patients account for roughly half of all medical costs. This expensive group is often an older and sicker population with multiple comorbidities which put them at higher risk for readmission. The inherent challenges of caring for this population are coupled with hospitals looking to reduce lengths of stay and a shortage of primary care physicians able to meet demands for visits outside of private offices. IHMG’s SNFist program helps post-acute care facilities meet these challenges while lowering readmission rates and improving patient satisfaction.

Our team of nurse practitioners serves as additional primary care givers within the four walls of the post-acute care facility and sees patients — whether they are permanent residents or are in intermediate convalescent care.

  • IHMG can provide physicians for Medical Directorship or work with your facility’s PCP or Medical Director.
  • SNFists are in a building up to seven days per week, working with the facility’s primary care providers to see patients in a timely fashion and enhance communication.
  • New admissions are seen quickly upon arrival, eliminating the uncertainty associated with predicting the availability of care providers.
